Installation Checklist & Warranty Activation
Submit a Warranty Checklist.
Complete and submit this form within 3 business days of reinstall. We'll email a completed copy to you and issue your Warranty Certificate within 2 business days of receipt.
Cooler REPLACED with new OEM or aftermarket unit —OR— machine hot-flushed (circle one completed). Proof of service ATTACHED to this form.
All cooler lines flushed with transmission flush solution and compressed air; replaced if cracked, collapsed, or internally corroded.
Engine rear main seal inspected — confirmed dry. (If replaced, note in Section C.)
Transmission wiring harness and all connectors — free of damage, corrosion, and chafing.
Driveshaft U-joints or front CV axles — inspected and confirmed serviceable.
Crossmember, mounts, and fasteners — inspected and confirmed in good condition.
Correct fluid type per vehicle manufacturer spec used. Correct quantity filled.
Engine cooling system confirmed free of coolant leaks. (Coolant contamination in cooler voids warranty.)
Shift linkage or cable adjusted per vehicle manufacturer specification after installation.
Post-install test drive completed — no abnormal shifting, slippage, or fluid leaks observed.
Vehicle scanned for transmission DTCs — no active codes found. (List any codes in Section C.)
Customer advised to return to install shop within 400 miles / 15 days for no-charge post-installation inspection.
Note part numbers, receipt reference, or flush machine used.
List any DTC codes found, exceptions noted, or additional context for this installation.
By signing below, the authorized representative of the Installing Shop certifies that all items herein have been completed as initialed; the rebuilt unit was installed in compliance with the Elite Transmission Rebuilders Limited Warranty Policy; all information on this form is accurate; and the Installing Shop has read and agrees to all warranty terms. The Installing Shop understands that failure to comply with any warranty requirement may void coverage and that the Warranty Certificate will be issued only upon receipt of this signed form.
